ImageIn the event of an injury at work, please ensure that within 24 hours of the incident that you contact the Direct Line for Reporting Injuries at KPRDSB 760-8670 or 1-877-577-7408 (ext. 3333), even if no medical attention is sought.  You should also report all injuries to your adminstration as well.

In addition, be sure that you fill out a WSIB form (Form 6) if you are injured at work, even if you do not miss any work days immediately after the incident.  Symptoms of the injury may occur later and filling out the forms may provide you with some coverage.  There are also WSIB forms for your administration (Form 7) and attending physician (Form 8) to fill out.  All of these forms are available at your worksite.  You can go to the WSIB website for more details.

In the event of a critical injury* as defined by the OHS Act, certain steps must be followed including notification to specific parties.  In such a case, contact MaryJane Smith, HR Specialist (Occupational Health & Safety) at extension 2219.

If an ambulance is called or if you are taken to the hospital and are unable to call, please have a colleague make the phone calls to report the circumstances as soon as possible.

Once the initial paperwork is complete, the employee may be contacted by Meg Hall, Disability Specialist (ext. 2222) or Ann Pritchard, Disability Specialisty WSIB (ext. 2084) for return to work and/or rehabilitation as necessary.
